Requesting online access to medical records Posted or Updated on 6 Jul 2016

Please note that the surgery's policy to grant access to any aspect of a patient's medical records requires a visit to the surgery with identification.

To secure access to appointment booking, medication requests and Summary Care Records a form needs to be signed and photo identification produced to allow the surgery team to ensure that only the registered patient (or patient with parental responsibility for a child under 16) gains access.

Access to 'detailed coded records' can take up to 28 days to be granted as each request is reviewed by a GP.

Access to full medical records is NOT available through Dorridge Surgery at this time

"SystmOnline" contradicts the above, which unfortuantley is out of the surgery's control.

Any request received through our website/online clinicial system will NOT be acted on & contact will NOT be made with patients to let them know.

If you do wish to access your available parts of your records online please visit the surgery with either passport or driving licence (or birth certificate for children that do not yet have a passport).
