Requesting additional online facitlities Posted or Updated on 10 Feb 2016

If you would like to add any of the following to your existing online access, please visit reception, as we do need you to sign a declaration for additional access:

• medication
• appointments
• access to summary care records

If you do require access to your Summary Care Record (ONLINE), we need photo ID as well.

The surgery does not subscribe to the option of using your current online access to request an upgrade, so please note that if you have requested any upgrade this will NOT be actioned & you will need to visit reception.

Access online to your medical records will be available from the 31st March, will require production of ID and completion of a form after reading a leaflet regarding what this means to you. ONLY  'detailed coded' records will be available online.
