CHANGE MY DETAILS?
If you move, please
let us know your new address as soon as possible so that we can keep our records
up to date. This is very important in case we need to contact you. Please check
with reception that your new address is still within our practice area. If you
change your name please also inform us as soon as possible in order that we may
update our records.
ORDER
REPEAT PRESCRIPTIONS?
Information regarding repeat medication
is stored on computer. Every time a prescription is issued a tear-off slip is
attached showing the names and dose of the repeat medication. This slip may be
used to order repeat prescriptions by ticking the items required then handing
it in or posting it with a stamped addressed envelope for return. Please read
any messages at the bottom of the tear-off slip.
Repeat prescriptions will be available 48 working hours after being requested. No telephone requests can be accepted for repeat medication unless in exceptional circumstances; this is to eliminate the possibility of errors.
It is important that patients on computerised repeat medication be reviewed by their doctor at least once every six to twelve months.
