The National Data Collection of all GP data on patients records is now scheduled to take place on the 1st September 2021. It is currently on an automatic 'opt in' basis, so if you DO NOT wish your records to be extracted then please complete the opt out form BEFORE the 1st September & return to the surgery. Thank you
