Please note you can ONLY register in person at the surgery. Although 'Systemonline' alludes to it being an option to register with the surgery using the online system, Dorridge Surgery does not use this facility.
When you register you will be registered as a patient of the practice. Please see reception who will check you are within practice boundaries, then provide you with registration forms & guidance on booking in for a New Patient Check. When you bring back your completed registration form/s, please provide the receptionist with an up to date form of identification (such as a valid passport). This will allow us to ensure your records transfer across accurately, & smoothly. At registration you may express the wish to be registered with a preferred GP. If you wish to change your usual GP at any time please speak to a receptionist. The practice will endeavour to comply, although it might not always be possible. If this is the case, an explanation will be offered.

We will only accept new patients who live in our practice area.
Where existing patients change address and move out of the area the surgery will give 30 days notice to re-register with a GP close to the new address.
